Becoming A Client
Registration for Community Transport is subject to an eligibility assessment.
For those aged over 65 years or 50 years for Aboriginal or Torres Strait Islanders, eligibility is determined by the My Aged Care (MAC) assessment team. You may contact MAC by phoning 1800 200 422 or online at http://www.myagedcare.gov.au/. MAC will do an initial assessment over the phone and will also arrange a full assessment in your home. If MAC determines that you are eligible for Community Transport service they will forward a referral to us and we will send you out an information package. Please remember to request Community Transport Port Stephens to be your chosen service provider.
For people aged under 65 years, or 50 years for Aboriginal or Torres Strait Islanders, who have a disability, eligibility for Community Transport services is determined by the National Disability Insurance Scheme (NDIS). You may contact the NDIS on 1800 800 110 www.ndis.gov.au.
If a person wishes to use Community Transport Port Stephens, but does not fit into the above categories, they may still be eligible for transport assistance. For enquiries please contact the office on 4984 7999.
